Artificial Intelligence and the Art of Smile Design

Artificial intelligence is expanding how we visualize and communicate smile transformations. Instead of imagining the final result, patients can see how subtle changes to tooth shape, alignment or position might influence their overall appearance.
But AI visualization is not just a communication tool—it can also strengthen our diagnostic thinking. When a digital platform generates a proposed smile design, it is up to us as clinicians to critically analyze what the software suggests. By reviewing AI-generated proposals, we can identify the changes being recommended, assess biological and orthodontic limitations, and determine whether the proposed design aligns with facial harmony and long-term oral health.
This analytical approach is central to my work in smile design and forms the foundation of my book, Smile Analysis. In it, I outline a practical framework for evaluating smiles that moves beyond a single-tooth mindset toward comprehensive, full-mouth care. The focus is on the art of observing and communicating smile aesthetics while guiding clinicians through structured treatment planning and case presentation.

I advocate a step-by-step approach that can be revisited and refined as skills develop. This method encourages analyzing the smile in the context of the entire face and communicating proposed changes effectively with patients.
Artificial intelligence can complement this process by generating visual proposals that stimulate clinical discussion and deeper analysis. When used thoughtfully, these tools help both clinicians and patients understand how dental changes influence facial balance and overall aesthetics.
Of course, technology must always be applied intentionally. AI-generated images can be compelling, but responsible treatment planning requires balancing aesthetic goals with biological limitations and facial harmony. The goal is not perfection—it is designing smiles that are natural, healthy and authentic to each patient.
This is the focus of my upcoming presentation, Artificial Intelligence and Smile Design, at the Wellness Dental Forum on October 17–18, 2026. In this session, I will explore how AI visualization tools can enhance clinical analysis, improve patient communication and support a more comprehensive approach to smile design.
In an era of rapidly advancing digital tools, I believe the future of smile design lies in combining technology with structured clinical analysis. When we learn to see smiles with intention and clarity, we can create treatment outcomes that support both oral health and the confidence that comes with a natural, harmonious smile.
While technology can enhance the process, meaningful smile design ultimately begins with how we observe, interpret and analyze what we see. Developing this skill set requires a structured framework that clinicians can apply consistently across cases.

The program begins with a three-hour live course on Saturday, September 12, introducing the foundational framework that underpins the full Smile Analysis System.
During the live session, I'll demonstrate how to observe, interpret and organize clinical information. Participants learn to analyze smile design cases using a macro, mid and micro framework, identify key aesthetic parameters and apply guided observation to their own cases.
The full course includes additional modules focused on defining treatment goals and mapping the path to achieve them.

Dr. Elaine Halley graduated from the University of Edinburgh in 1992 and became a member of the Royal College of General Dental Practitioners in 1995. She is a founder and past president of the British Academy of Cosmetic Dentistry and an accredited member of the International College of Dentists.
She earned a Master of Science in Restorative and Aesthetic Dentistry with distinction from the University of Manchester. She is a Digital Smile Design (DSD) Master, accredited by Christian Coachman, and serves as an official DSD instructor. She teaches Digital Smile Design as part of the Tipton Training Level 7 Postgraduate Certificate for dentists seeking to advance their clinical skills, with courses held in Watford, Manchester, and Dubai. She is also a member of Align Technology’s Global Faculty and teaches Comprehensive Invisalign Certification courses throughout the UK.
Dr. Halley serves as an Opinion Leader for multiple dental companies, including DMG, Ivoclar Vivadent, Optident, and Align Technology (Invisalign), and lectures widely across the UK, Europe, and the United States. She has authored numerous articles on clinical dentistry, smile design, treatment planning, digital dentistry, and the business of dentistry, and she serves on the board of the Digital Dental Society UK.
Elaine opened Cherrybank Dental Spa in Perth in 1995 and a second practice in Edinburgh in 2010, which she later sold to the Pain Free Dentistry Group. She currently serves as Clinical Director for the Pain Free Dental Group, mentoring associates across Scotland, many of whom visit the Perth practice to observe best practices. Cherrybank Dental Spa Perth remains an independent, private practice.
